Vaginitis is a collective term for various (infectious) disorders of the vagina, characterized by altered discharge. Vaginitis includes: 1) candidiasis; 2) trichomonas and 3) bacterial vaginosis. Below, each syndrome is further explained.
Candidiasis
Candidiasis or candida is an infection of the vagina caused by a yeast-like fungus called Candida albicans. Candidiasis is a common problem in women. On average, 25 per 1,000 female patients are diagnosed at the family doctor’s office each year.
Trichomonas
Trichomonas is an infection of the vagina caused by a parasite called Trichomonas vaginalis. Trichomonas is a sexually transmitted disease. With an incidence of 0.3 per 1,000 female patients per year, Trichomonas is a much less common problem than Candidiasis. Nevertheless, it is important to make the correct diagnosis because complications can occur especially in the long term or during pregnancy.
Bacterial vaginosis
Bacterial vaginosis is not an infection but an imbalance in the bacteria present in the vagina. In a healthy vaginal flora, bacteria are present in a certain proportion. In particular, lactobacilli must be present in sufficient quantities. This is because these bacteria create the low pH (high acidity) of the vagina, preventing infections. When too few lactobacilli are present, other bacteria take over and can cause infection. Bacterial vaginosis is a very common problem among women. Unfortunately, the exact numbers are not known because not every woman with this problem goes to the doctor.
Candida is a fungus that is present in the vagina in normal conditions in part of the female population and thus belongs to the normal flora. In some cases, however, Candida causes an infection. There is usually no obvious reason for this, but in some cases a cause can be identified, such as a compromised immune system, pregnancy or an underlying condition such as diabetes.
Trichomonas can be transmitted during unprotected sex through the mucous membranes of the genitals, secretions, pre-cum and semen.
It is not entirely clear why some women frequently suffer from bacterial vaginosis and others do not. However, it is clear that symptoms can be worsened by unprotected sex because semen affects the acidity in the vagina.
Bacterial vaginosis
The typical complaint in bacterial vaginosis is an increased amount of discharge that is gray in color and smells unpleasant.
Fungal infection/candida infection
Typical of candida is a thickened, white and friable discharge that smells no different than normal. Other possible symptoms include burning, itching and redness of the vagina.
Trichomonas
A typical symptom is increased discharge, which may be yellow-green in color and smell unpleasant. Furthermore, a red vaginal wall is often noticeable and the cervix may contain dark red dots upon inspection (this is also called a strawberry aspect).

The main difference between bacterial vaginosis and a fungal infection is that with bacterial vaginosis, there is no painful, swollen or red mucous membrane. There is also no pain during urination or intercourse. A vaginal yeast infection usually does. Thereby, itching is also a characteristic symptom for a fungal infection, which is not so in bacterial vaginosis. Furthermore, discharge that is white, odorless and friable is a hallmark of a vaginal yeast infection. In bacterial vaginosis, the discharge may smell strongly and be grayish-white in color.
You can recognize bacterial vaginosis by changes in vaginal discharge such as in quantity, odor and/or color. Complaints of itching or burning in the vagina. Any complaints of redness of, for example, labia.
A bacterial vaginosis need not be treated in principle. Only in cases of very bothersome symptoms can the doctor prescribe a course of metronidazole. Local treatment is preferred. Alternative treatments, such as vaginal washes, have not been proven effective.
Some women appear to be more susceptible to bacterial vaginosis. But the reason is not yet known. It has been proven, however, that unprotected sex can worsen symptoms because semen affects the acidity of the vagina.
Typical symptoms of a vaginal yeast infection or candida is a vaginal discharge that is thickened, white and friable, but still smells normal. Itching, redness and a burning sensation also accompany this.
Candida can be easily treated. Treatment is not necessary, but is advised for bothersome symptoms. In general practice, the drug of choice is often miconazole, an antifungal agent. Local treatment is preferred, with one capsule of miconazole to be inserted into the vagina.

After about two weeks, the symptoms are gone.
Typical of trichomonas is more discharge, which smells unpleasant and can be yellow-green in color. A red vaginal wall and dark red dots on the cervix are also known symptoms.
Trichomonas is usually treated with metronidazole, a medication you have to take once. In case of pregnancy (in the first trimester), alternative treatment will be offered.
Trichomonas can be transmitted during unprotected sex through the mucous membranes of the genitals, secretions, pre-cum and semen.
